Person-Centred Planning is an approach that places the student at the centre of goal setting. Students and their families have an opportunity to participate in three Person-Centred Planning programs at key points of transition in their high school pathway.
Plan@bility is a strategic and dynamic planning tool designed to put the people involved at the centre of the process, allowing them to plan for their future in a way that is meaningful to them.
The Project- has 3 planning sessions at 2 hours per session. These sessions will encourage everyone involved to plan. The outcomes will form part of our plan for a successful transition for students in years 12 and 13.
The Process- creates an environment where individuals, families and support people can imagine and collaboratively plan for a future where they are fully included in the lives of their community. It utilises support people to maximise their potential and bring their plan to fruition. It focuses on strengths, interests, hopes and aspirations.
The Outcome- to raise awareness of the critical importance of transition planning using Plan@bility as a workable and valuable tool for staff and more importantly students, families and supports.
